What to Expect During Your Brazilian Butt Lift Surgery
At our AAAHC-accredited facility in Fall River, Dr. Mark X. Lowney performs the “SAFER BBL” procedure, or a less invasive surgery than the traditional Brazilian Butt Lift, by substituting general anesthesia for local anesthesia using IV Conscious Sedation and tumescent liposuction.
This approach results in youthful, pronounced, perky buttocks for patients who want to improve their flat bottoms or accentuate their natural curves. SAFER BBL combines two cosmetic procedures: Lipo360 and fat transfer to the buttocks. Lipo360 consists of liposuction of the entire “core,” which includes the upper and lower abdomen, hip roll areas (or love handles), and upper bra bulge areas (fat underneath the bra). Next, the fat is purified and immediately reinjected into the buttocks. These two procedures result in:

How Dr. Lowney’s SAFER BBL is Unique
Dr. Lowney has performed over 1,000 procedures utilizing the SAFER BBL technique. Most Lipo360 procedures take three to four hours to perform. The actual fat transfer to the buttocks takes about 30 minutes, so on average, it is a four-hour procedure. With proper fat harvesting and reinjection techniques, fat transfer to the buttocks should last forever. Sometimes, patients can lose up to 30% of fat within the first six months after surgery. Dr. Lowney’s SAFER BBL incorporates the following principles:
Never Injecting Fat Into or Under Gluteus Muscles
Medically, this is known as autologous fat transfer to the subcutaneous. Dr. Lowney only uses the patient’s fat between the skin and the gluteus muscle layer. This technique avoids injecting fat into blood vessels in the muscles, which can lead to a lethal fat embolism.
Using a Larger Infiltration Cannula
A larger-sized cannula places the fat into the subcutaneous layer of fatty tissue, thus decreasing the risk of lacerating (cutting) a blood vessel, which can lead to fat embolism.
Using a Power-Assist Device
This advanced technology vibrates the infiltration cannula during tunneling and fat infiltration into the subcutaneous space. Again, this helps the surgeon make tunnels (potential spaces) in the adipose (fatty) layer between the skin and muscle. Creating these tunnels makes it easier to place fat more safely.
Incorporating a Fat Transfer Infiltration System
This system allows professionals to regulate the pressure of fat entering the subcutaneous space. By infiltrating the fat at low pressure (i.e., not forcing it into the subcutaneous space) and not “overfilling “the space under high pressure, the chance of placing fat into the muscle decreases.
Placing the Incisions Strategically
Adits are small incisions where the infiltration cannula is placed. They are carefully placed above and below the buttocks and directed away from the “danger zones” where large blood vessels are located in the muscle.
- Autologous (using the patient’s fat) fat transfer to the subcutaneous layer ONLY (space between the skin and gluteus muscle), NEVER injecting fat into or under the gluteus muscle. This technique avoids injecting fat into blood vessels in the muscles, which can lead to a lethal fat embolism.
- Utilization of a large-sized infiltration cannula (#4 or greater) to place the fat into the subcutaneous layer of adipose tissue, thus decreasing the risk of lacerating (cutting) a blood vessel which can lead to fat embolism.
- Utilizing a power-assist device that vibrates the infiltration cannula during tunneling and fat infiltration into the subcutaneous space. Again, this technology helps the surgeon make tunnels (potential spaces) in the adipose (fatty) layer between the skin and muscle. Creating these tunnels makes it easier to place fat more safely.
- Utilizing a fat transfer infiltration system, you can regulate the pressure of the fat going into the subcutaneous space. By infiltrating the fat at low pressure (i.e., not forcing it into the subcutaneous space) as well as not “overfilling “the space under high pressure, this decreases the chance of placing fat into the muscle
- Placing the adits (small incisions where the infiltration cannula is placed) in strategic locations above and below the buttocks and directing the cannula away from the “danger zones” where large blood vessels are located in the muscle.

What to Expect After BBL Surgery
Before your procedure, you will speak in-depth with Dr. Lowney and receive specific recovery instructions to ensure you heal swiftly and see maximum results as soon as possible. After your BBL procedure, Dr. Lowney will wrap the affected area with a compression band to reduce bleeding. If you receive general anesthesia, the anesthesiologist will stop the flow of anesthesia into the body. Although the Lipo 360 part of the procedure will cause some soreness, the actual fat transfer site of the buttocks remains relatively pain-free.
You will then move to a recovery room. Once awake, you may feel dizzy and sore. Dr. Lowney will monitor your overall health for several hours post-op. Once they determine you no longer need monitoring, you will be discharged. Most cosmetic surgery patients are sent home on a “narcotic-free” medication regimen. A family member or friend should drive you home and take care of you for 24 hours.
Key Factors Influencing BBL Recovery Time
Most patients need one drain placed across the lower part of their back, as this area can accumulate fluid and form a seroma. These drains are usually removed in five to seven days. Depending on the type of work, most patients can resume employment in one to two weeks. It’s normal to return to your routine, including sexual activity, within two to three weeks.
It may take up to six months after your BBL before you are fully recovered. You should eat a high-protein diet to ensure optimal healing during this time. This diet also helps to maintain your new shape. You may also have to slowly begin doing lower body exercises focusing on the buttocks, including leg lifts, lunges, and squats.

Getting a Brazilian Butt Lift is considered an elective cosmetic surgery, which means it is a personal decision. If this is a surgery you want for your body, consider if you are the proper candidate. HOW MUCH FAT CAN BE PLACED INTO THE BUTTOCKS?
On average, approximately 750cc to 1500cc is placed into each buttock.
IF I GAIN OR LOSE WEIGHT, WILL IT AFFECT THE BRAZILIAN BUTT LIFT?
Because the transferred fat is living tissue, if you lose weight, the fat cells will shrink, and so will the lift. If you gain weight, the fat cells will increase in size and accentuate the lift.
Healthy lifestyle choices, such as eating a primarily whole-food diet, doing at least 20 minutes of aerobic exercise daily, and refraining from smoking, will optimize the results.
AFTER MY SURGERY, HOW OFTEN DO I NEED TO COME IN FOR POSTOP CARE?
Patients are seen within 24 hours of surgery and then one week later for drain removal. If you are a local patient, you are seen one month and six months later for check-ins.
HOW LONG DO I NEED TO WEAR MY COMPRESSION GARMENTS AFTER HAVING A BBL?
Patients are advised to wear custom-made compression garments for at least 4 to 6 weeks postoperatively.
WHEN CAN I SIT ON MY BUTTOCKS AFTER HAVING A BBL?
We recommend that patients refrain from sitting on the buttocks after a BBL for at least two weeks. There are special “boppy” pillows you can sit on during this period if sitting is required.
